LGSUHSD Parents Volunteer Upcoming Sign-Up Events
We will be hosting a volunteer event for parents interested in volunteering for the 2022-23 school year at each school site this month. These events are not site specific, you can come to either date to apply to be a volunteer.

At the bottom of the form, please attach:
- Driver Insurance (If you plan to drive students)
- Negative TB Results ( Within the last 4 years) If you DO NOT have an updated TB test there will be a registered nurse at the event to do a TB risk assessment. If you would like to get the risk assessment completed prior to the event, please complete this form and our district nurse, Lisa Tripp, will be in touch with you. Fingerprint Information: We will have a fingerprint station at the event.
- Volunteer Fingerprints cost $47 and your payment is appreciated, but not required.
- Should you chose to support district programs by contributing the cost of fingerprinting:

Notes:
- You must still get fingerprinted to volunteer for the LGSUHSD even if you were cleared to volunteer at your students' middle school.
- Please make sure that all of the documents required above are attached to your volunteer packet. You will not be cleared unless they are included.
